Guest and Visitor Agreement and Assumption of Risk

**IF YOU ARE A MEMBER, PLEASE FILL OUT THE ‘MEMBER AGREEMENT AND ASSUMPTION OF RISK’ FORM**

This form must be completed each time you visit Ladies’ Golf Club of Toronto. Failure to do so will result in denial of access to the property. This form will remain in effect until the Board of Directors of the club declare the requirements in this form are no longer in effect.

In addition, each time you arrive at the property, a greeter will be stationed at the driveway entrance. You must stop and answer mandated daily screening questions prior to gaining access to the property.

The club appreciates and expects your partnership and cooperation in allowing LGCT to operate given the guidelines and social distancing protocols put into effect by the provincial government. We all play by the rules of golf and now request that you observe our rules for operating under COVID-19 restrictions. LGCT is permitting play based on the assurances, as a guest or visitor, make to the club and to each other in this document. Accordingly, any false statement could result in legal liability to you and/or the club. Further, you agree to observe and fully abide by the rules and regulation prescribed by the club. Any failure in this regard may result in disciplinary action.

As you are aware, the virus can be transmitted by asymptomatic people and accordingly, the statements made by you contained in this declaration cannot provide certainty that the virus will not be transmitted. The club is taking prudent steps to impose and enforce appropriate protocols to keep guests and visitors safe, but there can be no assurance that the virus will not be contracted by players while at the club. This is a risk that each guest and visitor must assess themselves, and if they choose to play, to take the risk of either contracting the virus or transmitting it to others.

As you are aware, COVID-19 can be transmitted by people who do not have symptoms and accordingly the agreements made by you in this form cannot provide certainty that COVID-19 will not be transmitted at the club. Further, while the club is taking steps to impose and enforce health and safety protocols to reduce the risk of transmission of COVID-19, there can be no assurance that COVID-19 won’t be contracted by you visiting the club. This is a risk that each person must assess themselves, and if they choose to visit the club, they take the risk of either contracting COVID-19 or transmitting it to others. By choosing to visit the club, I acknowledge and accept that I am at risk of contracting COVID-19 and/or transmitting it to others and I voluntarily accept that risk. *

I agree that I will not visit the club if I or anyone in my household or anyone I have come into close contact with has tested positive for COVID-19, or is waiting for the result of a COVID-19 test, and has not been cleared by a medical professional to be in public. NOTE: Close contact includes living with, providing care, or otherwise having close prolonged contract (15 minutes and within 2 metres) with another person. *

I agree that I will not visit the club if I have any of the following symptoms: fever, onset of cough, worsening chronic cough, shortness of breath, difficulty breathing, sore throat, difficulty swallowing, decrease or loss of sense of taste or smell, chills, headaches, unexplained fatigue/malaise/muscle aches (myalgias), nausea/vomiting, diarrhea, abdominal pain, pink eye (conjunctivitis), runny nose or nasal congestion without other known cause. *

I agree that if I or someone from my household are required to self-isolate by the government after returning from travel outside of the Canada, I will not visit the club for the prescribed duration. *

If, within 14 days of my attendance at the club, I or a member of my household, or anyone with whom I have come into close contact with, tests positive for COVID-19, I will immediately notify Paul Doucet, Club General Manager (905-889-3531 x306) to allow the club to take appropriate actions and precautions. *

I have read, understand and will comply with the COVID-19 Health and Safety Policies as communicated by the Club and accept that I waive any right to visit the Club should I not comply with these policies. *
